FORM 1095
Per the Paperwork Burden Reduction Act (H.R. 3797) (“PBRA”), that was passed on December 11th, 2024, Employers are no longer required to send employees a Form 1095-B or a Form 1095-C. However, if you would like a copy of this form, please reach out to the payroll manager and office manager immediately. You can send an email to payroll@explore.academy and tina.kozlowski@explore.academy and include your name, email address, mailing address, and phone number.
Public Records Requests (IPRA)
To request any public records pertaining to Explore Academy - Albuquerque please contact the board liaison via email, using "Public Records Request" as the subject. A written request must contain the name, address and telephone number of the person making the request and must describe the records sought in sufficient detail to enable the records custodian to identify and locate the requested records. Requests must indicate if records will be used for commerical purposes. For electronic copies, please also include the email address.
